NTSB: Duke Life Flight pilot closed down wrong engine before fatal crash

— The pilot of a Duke Life Flight medical helicopter that crashed in northeast North Carolina more than 3 years back, killing all 4 individuals aboard, likely got puzzled by a selection of emergency situation indicators about an issue with among his two engines and erroneously shut down the other one, according to federal investigators.

In their final report on the crash, National Transport Safety Board investigators stated harmed roller bearings in the No. 2 engine likely triggered the engine to seize up throughout flight.

The damaged No. 2 engine then likely lost all power, according to the report, and the wreckage “showed that the helicopter was in a near-vertical descent
prior to affecting the ground.”

NTSB investigators could not identify the reason that the No. 2 engine failed since of a mix of damage from its continued operation and from the crash, as well as a subsequent fire.

Witnesses informed investigators that they saw a path of dark smoke originating from the helicopter prior to the crash, however the NTSB could not identify if the No. 2 engine was on fire during flight.

Investigators likewise noted that metallic pollutants were discovered in oil from the No. 2 engine during routine tests in the months prior to the crash, showing a potential bearing failure. However the amount of contamination never rose to the level specified by the engine manufacturer to necessitate action.

“The oil test examination treatments did not consist of steps to keep track of trends of impurity concentration levels over time.
